ALLERGIES-Due to Covid 19, we will not be suppling water and as many snacks as we usually do. We do have some individually packaged chips and candy that we give out on occasion, but we ask that you supply the bulk of your child's snack needs. Make sure your child has water for the afternoon. Please let us know if your child has any allergies or sensitivities. And if so, please list them below.

PICK UP AUTHORIZATION-Please list below all the names of the people authorized to pick your child up and their Contact #. If someone comes who is not on the list. I will have to call the parent/guardian’s # that we have in our records to get confirmation that the person allowed to pick up your child.

EMERGENCY CONTACT-Please let us know the pnone # of who to contact in case of an emergency

If you are using our AFTER SCHOOL PICK UP service for the first time, please upload a picture of your child. It makes pick up easier!
